Community Care Hubs
A Community Care Hub is a community-focused entity that organizes and supports a network of community-based organizations providing services to address health-related social needs. It centralizes many administrative functions and operational infrastructure.
Beacon was one of twelve initial pilot sites awarded to enhance its operations into a Community Care Hub. Funding was provided by the US Department of Health and Human Services’ Administration for Community Living. Beacon is currently the only Community Care Hub operating in Louisiana. Beacon currently operates many of its services and projects as Community Care Hubs.
Reentry Hub
Comprehensive Support for Successful Reintegration
As the Louisiana Department of Corrections’ contracted partner, Beacon Community Connections delivers vital Community Incentive Grants in Lafayette and Calcasieu parishes. Our Reentry Hub focuses on providing pre- and post-release support to individuals at risk of re-incarceration.
We work proactively with participants before their release, ensuring they’re prepared for the transition. Upon release, we guide them to critical resources, including housing, food, transportation, and employment opportunities.

Nurture Network Hub
Empowering Mothers in Louisiana
Beacon Community Connections is leading a collaborative effort across Louisiana to transform maternal care. Our Nurture Network focuses on ensuring every pregnant woman and new mother receives comprehensive support before, during, and after childbirth. We’re committed to:
- Reducing maternal mortality rates.
- Increasing access to early prenatal care.
- Providing robust postpartum support.
As the only Community Care Hub in Louisiana, we uniquely connect healthcare, social services, and community resources. Through our Maternal Coordinated Care Network, we address the holistic needs of mothers, including physical, mental, and social well-being. We’re dedicated to eliminating healthcare disparities and creating a healthier future for all mothers and babies in Louisiana.

Reaching Vulnerable Older Adults through Collaboration
Beacon Community Connections is proud to be a 2025 AARP Foundation Capacity Building grant recipient. This grant enables us to establish a robust Community Care Hub, dedicated to serving vulnerable older adults. We’re focused on expanding our network of partner organizations and enhancing data interoperability to provide seamless, comprehensive care.
